Expressions of Addiction Privacy Policy

 

Expressions of Addiction is an exhibit of original photographic portraits that depict people in various stages and expressions of addiction. Each portrait includes a brief biosketch of the subject. This photography exhibition will help the public better understand addiction by reaching hundreds of thousands through gallery exhibitions, television programs, and Internet sites.

 

In consideration for value received, receipt whereof is acknowledged, I hereby give Howard Shaffer the absolute right and permission to copyright and/ or publish my photograph and biosketch, use photographic portraits or portraits or pictures of me, in composite or distorted character or form, in conjunction with my own or a fictitious name, or reproductions thereof in color or otherwise, made through any media at his studios or elsewhere, for art, advertising, trade or any lawful purpose whatsoever. I also give Howard Shaffer the absolute right and permission to copyright and/or publish the biosketch that is associated with my portrait.

 

I hereby waive any right that I may have to inspect and/or approve the finished photo-graph and biosketch product or the advertising copy that may be used in connection therewith, or the use to which it may be applied.

 

I hereby release, discharge, and agree to save Howard Shaffer from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said pictures, or in any processing tending towards the completion of the finished product.

 

I understand that this consent is perpetual; I may not revoke it and that it is binding on my heirs and assigns.

 

I warrant that I am at least 18 years of age and that I am competent in my own name insofar as this consent is concerned. I further attest that I have read this consent form and fully under-stand its contents.

 

In lay language, this consent means that you give Howard Shaffer the complete right to use your photograph and your biosketch as is necessary for this project. You give him permission to publish or use this material for the purposes of this educational project. You don’t have the right to review the portrait or the biosketch before it is used. Finally, you will receive a signed photograph for being involved.
